What is perimenopause?
Perimenopause is the hormonal transition that typically begins in the late 30s to mid-40s, when oestrogen and progesterone levels start to fluctuate before eventually declining. It can last anywhere from two to twelve years and is one of the most common - and most poorly supported - phases of women's health. Menopause itself is confirmed once twelve consecutive months have passed without a period.

Perimenopause doesn't announce itself with a single clear sign. Most people notice a cluster of changes: periods becoming irregular, heavier, or shorter; sleep becoming harder even when you're exhausted; mood shifts that feel out of proportion; and a general sense that your body is behaving differently. A thorough hormone and pathology assessment can help clarify where you are in the transition and what's driving your specific symptoms.

Naturopathic care and HRT are not mutually exclusive. Some people do well with naturopathic support alone; others benefit from both working together. A naturopath can support hormonal metabolism, adrenal function, sleep, gut health, and nervous system regulation - all of which influence how your body experiences the perimenopausal transition. The goal is always to find what actually works for your body, not to apply a one-size approach.

Depending on your symptoms and history, assessment may include a comprehensive thyroid panel, sex hormone testing (oestrogen, progesterone, testosterone, DHEA), cortisol and adrenal function markers, iron studies, vitamin D, and inflammatory markers. DUTCH hormone testing is used where a more detailed look at hormone metabolism is needed. Testing is always chosen based on what's clinically relevant for you, not as a blanket panel.

Hormonal recalibration takes time — the goal is sustainable change, not a quick fix. The pace depends on how long symptoms have been present, what other health factors are at play, and how consistently the treatment plan is followed.

It's not. Perimenopause can begin earlier than most people expect, particularly if there is a history of thyroid issues, high stress, significant hormonal disruption, or surgical menopause. If your cycles are changing or you're experiencing symptoms that don't have another clear explanation, it's worth investigating — earlier support tends to produce better outcomes.
